Successful implantation of autologous muscle-derived stem cells in treatment of faecal incontinence due to external sphincter rupture . By Michał Romaniszyn, Natalia Rozwadowska, Marcin Nowak, Agnieszka Malcher, Tomasz Kolanowski, Piotr Wałęga, Piotr Richter and Maciej Kurpisz.

The results show that the primary overlap technique in the repair of anal sphincter rupture following vaginal delivery is highly successful. Thirty‐eight women with rupture of the anal sphincter occurring during childbirth were followed for 3–12 months. Nineteen had complete rupture of the external and sphincter, 14 had a lesion involving more than half of the sphincter muscle and five had a superficial rupture. Fourteen patients presented with continence disturbances: nine to solid or liquid faeces and five to flatus When the surroundings are suitable defecation proceeding with rising of intra-abdominal pressure, relaxation of the puborectalis muscle with subsequent increase of the anorectal angle and relaxation of external anal sphincter to get rid of fecal matter, but when the surroundings are unsuitable voluntary contraction of external anal sphincter and puborectalis muscle leads to inhibition of rectoanal inhibitory reflex, relaxation of rectal wall muscles and subsequent backward colonic The sphincter muscle is involuntary. We do, however, have some control over the external sphincter.

It consists of two layers: superficial and deep . The external sphincter is a layer of voluntary (striated) muscle encircling the outside wall of the anal canal and anal opening. One can cause it to expand and contract at will, except during the early years of life when it is not yet fully developed. Nerves….

Anal sphincter rupture was 2.4 times as common as reported in Swedish birth statistics. The high incidence of fecal incontinence by 6 months postpartum in all women is surprising and deserves further investigation, specifically regarding occult sphincter rupture.

External sphincter rupture

It’s voluntary in nature and is furnished by the pudendal nerve (S2, S3, S4). Differences Between the Internal and External Sphincters of the Urethra Obstetric anal sphincter rupture (OASR) is a well‐known complication of vaginal deliveries, with a frequency of 0.5–5% reported in the literature (1). Between 20 and 67% of these patients develop fecal incontinence, which has great impact on quality of life as well as subsequent pregnancies (2). 11 of 15 patients with external sphincter defects and 6 of 8 patients with combined external and internal sphincter defects that have undergone surgery and contact with the surgeons after surgery for the operative results revealed an excellent correlation between the operative findings and 3D ultrasonography, with respect to the site, the shape, and extent of the defects except for 1 case with the external sphincter, this muscle also contracts and causes the angle which is present in the rectum to sharpen, therefore decreasing the chance of incontinence. Any of these muscles can become weak, which means the anal canal does not close fully and so leakages may occur. Concurrent and frequently more severe life-threatening injuries may mask the rupture and it may only become apparent when the owner notices a swelling, or secondary signs due to strangulation of herniated organs occur.

In males, the internal sphincter muscle of urethra functions to prevent reflux of seminal fluids into the male bladder during ejaculation.. Though resting sphincter tone is predominantly attributed to the internal anal sphincter, studies under general anesthesia or after pudendal nerve block suggest the external anal sphincter generally accounts for approximately 25%, and up to 50%, of resting anal tone. 9, 33, 40, 50, 144, 170 When continence is threatened, the external sphincter contracts to augment anal tone, preserving continence.

For partial thickness tear of the external anal sphincter the end-to-end technique is recommended. √ The muscle ends of the external anal sphincter should be clearly identified to reconstruct the sphincter in full thickness and height.
